There is a way indeed. Just click on the "link" link within the comment's header. On the standalone comment page there will be a "flag" option within the comment header. There may be a karma limit or something though; I'm not sure about that.
I was using Google Reader, and there was no comments link, but have since switched to NetNewsWire (OS X) where everything works correctly. It doesn't look like it'll change.
You can, but HN hides it (presumably to prevent abuse). Go to the individual item ID (click the time of the comment) and there's a "flag" hyperlink:

[A:] You can't. This is to prevent people from using this method as a way of submitting a link, but with their comments in a privileged position at the top of the page. If you want to submit a link with comments, just submit it, then add a regular comment.
I'm a bit puzzled as to how this misunderstanding arose, and it seems like it might be a UI issue. Perhaps it was because the actual flagged comment was collapsed, so it looked like the [flagged] annotation was somehow stuck to yours?
It's done that way as a speed bump to discourage reflexive flagging. An old PG trick.
